Joe Marchiano, PharmD, BCPS, BCGP is an Assistant Professor of Pharmacy Practice at Northeast Ohio Medical University and Clinical Lead Pharmacist in the specialty of Geriatrics at Summa Health. He completed a PGY-2 residency in Internal Medicine and Academia through the University Hospitals Geauga Medical Center and Northeast Ohio Medical University and a PGY-1 residency at the UH Geauga Medical Center. He currently practices on ambulatory and inpatient multidisciplinary geriatrics teams and enjoys maintaining professional advocacy.
Nancy MacDonald, PharmD, BCPS, FASHP is the Coordinator of Transitions of Care and Emergency Preparedness at Henry Ford Hospital and Adjunct Faculty for Wayne State University Eugene Applebaum College of Pharmacy in Detroit, MI. She currently serves as the Chair Elect for the ASHP Section of Clinical Specialists and Scientists. Dr. MacDonald’s interests include transitions of care, health equity, advancing clinical pharmacy practice, and leadership development.
Phuoc Anne Nguyen, PharmD, MS, BCPS, FTSHP is proud native Houstonian where she received her Bachelor of Arts in Biology and Doctor of Pharmacy degree from the University of Texas at Austin. She completed a PGY1/PGY2/MS in Health Systems Pharmacy Administration Residency at Michael E Debakey VA Medical Center and obtained a Master’s degree in Pharmacy Administration from the University of Houston College of Pharmacy. After residency, she worked for several years as a clinical pharmacy specialist in transitions of care and internal medicine at the University of Texas MD Anderson Cancer Center. She is currently working as a pharmacy manager at Houston Methodist West hospital, assisting with operational and clinical initiatives. Her professional interests include leadership developments, transition of care initiatives, public health and patient safety.
Brooke Barlow, PharmD is a clinical specialist in the neuromedicine intensive care unit at UF Health Shands Hospital and serves as a clinical assistant professor at the University of Florida College of Pharmacy. Dr. Barlow received her Doctorate of Pharmacy from Jefferson College of Pharmacy and completed her PGY1 Pharmacy Practice Residency and PGY2 Critical Care Pharmacy Residency at University of Kentucky Healthcare. She is active within multiple professional organizations including NCS, ACCP, SCCM, and ASHP’s New Practitioners Forum Clinical Practice Advisory Group.
Snehal H. Bhatt, PharmD, AACC, BCPS-AQ Cardiology, FASHP is Professor of Pharmacy Practice at the MCPHS University, and Clinical Pharmacist at the Beth Israel Deaconess Medical Center, in Boston, Massachusetts. He teaches and practices in cardiology and anticoagulation. As an active member of the American Society of Health-System Pharmacists (ASHP), Dr. Bhatt is currently a content matter expert and faculty member in ASHP’s BCPS recertification and preparatory course, and represented ASHP on the Joint Commission Technical Advisory Panel for National Patient Safety Goal 03.05.01-Reducing Harm from Anticoagulant Therapy: Update to Include Direct Oral Anticoagulants (DOACs). He also served as chair on the ASHP Council on Therapeutics.
Elyse MacDonald, PharmD, MS, BCPS is the assistant director of pharmacy services focusing on research pharmacy at Stanford Health Care. Prior to this position, she was the pharmacy manager at the Investigational Drug Service with University of Utah Health. She trained in Drug Information at the University of Utah Health. She is a current member of the Clinical Leadership Section Advisory Group as part of the Section of Clinical Specialists and Scientists for ASHP.
Hai Tran, PharmD, BCPS is currently the associate director of Drug Use Policy, Antimicrobial Stewardship and Clinical Pharmacy Prior Authorization at Cedars-Sinai Medical Center. In her current role, she is responsible for formulary management, leads the hospital pharmacy clinical programs, develops and implements cost saving strategies in collaboration with Medical and Nursing Leadership. Additionally, she is also responsible for the revenue retention and recovery for high cost/ target drugs.
Brittany Tschaen, PharmD, MS is a clinical pharmacist at ChristianaCare, DE. After receiving her Doctor of Pharmacy from Jefferson College of Pharmacy, she completed a PGY1 pharmacy practice residency at Penn Presbyterian Medical Center and a PGY2 residency in internal medicine at NewYork-Presbyterian Hospital. She is currently serving as an ASHP New Practitioners Forum (NPF) executive committee member and liaison to the NPF Residency Advancement Advisory Group (RAAG).
