Maintaining the Recommendations
Goal
To assure that the Standardize 4 Safety recommendations remain current and continue to meet the needs of patients and the health care practitioners and organizations that take care of them.
With significant resources invested in creating, implementing, and maintaining the Standardize 4 Safety recommendations to achieve the vision of a national standard, ASHP is committed to a public and multi-disciplinary approach to maintaining the standards.
Standardize 4 Safety Advisory Panels: Adult and Pediatric
An advisory panel, composed of members of the expert panels who created the Standardize 4 Safety lists, meets quarterly to address any requested or recommended changes in the standards, review adoption of the medication concentrations, and other Standardize 4 Safety needs.
Panel members are multi-disciplinary healthcare practitioners from stakeholder organizations and will serve 2- or 3-year terms. Terms renew in August of each year.
Recommendations for Standardize 4 Safety Lists
Any advisory panel member or public stakeholder may make a recommendation to update, add or delete medications, or other changes to the existing standards. The requests may be submitted by completing this on-line form.
Updated Changes for Standardize 4 Safety Lists
Updates and changes to the Standardize 4 Safety lists, when recommended by the advisory panels, will occur the third week after the advisory panel meetings in September, December, March, and June.
Communication of changes and topics under current deliberation by the advisory panels will occur on the S4S Connect community quarterly.
