Within the UK HealthCare system, Albert B. Chandler Hospital is a 1,040-bed tertiary care academic medical center located in Lexington, Kentucky. Recent Significant Projects:
Some of the most exciting projects we've worked on recently to improve care for diabetes patients include supporting patients diabetes management during transitions of care through a pharmacist-managed diabetes transitions of care clinic, working with Meds-to-Beds to create a diabetes discharge order set, partnering with our new durable medical equipment team to improve access to diabetes technology, and working with our retail and specialty pharmacy teams to improve medication access. Being involved with residency research projects and medication use evaluations has helped identify outcomes for some of our system approaches for improving care for people with diabetes both in the hospital and outpatient settings.

Initial Involvement in ASHP:
I have been a member of the ASHP Section of Ambulatory Care Practitioners Advisory Group on Clinical Practice Advancement for the past four years.

Why did you become involved in ASHP?
I originally became an ASHP member as a resident to plug into career opportunities and the network of residency programs. I decided to join the SAG for Clinical Practice Advancement to contribute to the leadership in ambulatory pharmacy and support my colleagues in growth in our area.Advice for Someone New to Specialty Area Diabetes management has exploded lately with new drugs, new indications, overlap with other chronic disease states, and technology. Becoming an expert in this area requires a lot of frequent updates, but is valued by many healthcare settings as such a large population is affected by diabetes. Identifying opportunities for a pharmacist to practice to their strengths in this area, as well as opportunities to collaborate across disciplines and specialties can provide opportunity for novel practice models.

How would you explain the value of ASHP to a friend or colleague?
As a residency program director, ASHP provides guidance and community for optimizing our residency programs. As an ambulatory care pharmacist, the resources provided by ASHP are valuable in practice development and growth. The SAG's education sessions, webinars, and roundtables really support growth and idea sharing to progress the profession.

What is the value of ASHP for the profession?
ASHP helps unite pharmacy professionals in health systems by providing education, advocacy, facilitating connections, and leadership. By pushing the profession forward, they ultimately support healthcare system improvement and optimizing patient outcomes.
