SPEAKERS

Jennifer ClementsJennifer N. Clements, PharmD, FCCP, BCPS, CDE, BCACP, is Clinical Pharmacist in Diabetes Transition with Spartanburg Regional Healthcare System in Spartanburg, South Carolina. 

Dr. Clements earned her Doctor of Pharmacy degree from Campbell University and completed a primary care pharmacy practice residency at the Ralph H. Johnson VA Medical Center (VAMC) in Charleston, South Carolina.  She is a board-certified pharmacotherapy specialist, board-certified ambulatory care pharmacist, and certified diabetes educator.  She has developed and implemented pharmacist-led services in a Veterans Affairs Medical Center and rural-health, family medicine office.  Currently, she is developing a pharmacy service, focusing on inpatient management of diabetes.

Dr. Clements is a member of American Association of Diabetes Educators (AADE), American College of Clinical Pharmacy (ACCP), American Society of Health-System Pharmacists (ASHP), South Carolina Society of Health-System Pharmacists (SCSHP), and South Carolina Pharmacy Association (SCPhA).  She became a Fellow of ACCP in 2017.  She is the Past-President for SCSHP and Past-Chair for the ACCP Ambulatory Care Practice and Research Network.  She serves as a member of the ASHP Ambulatory Care SAG Educational Committee.

Zach WeberZach Weber, PharmD, BCPS, BCACP, CDE is the Director of Interprofessional Education and Clinical Associate Professor of Pharmacy Practice for Purdue University College of Pharmacy and the Assistant Dean for Programming with the Indiana University Interprofessional Practice and Education Center.  He received his PharmD degree from Purdue University and completed a PGY1 Pharmacy Practice residency at the Roudebush VA Medical Center in Indianapolis, IN followed by a PGY2 Ambulatory Care Specialty residency at Duke University Hospital in Durham, NC. He is a board-certified pharmacotherapy and ambulatory care specialist, and a certified diabetes educator. Dr. Weber is a past Chair for the American Society of Health-System Pharmacists (ASHP) Section Advisory Group on Compensation and Practice Sustainability and is the current Chair for the ASHP Section of Ambulatory Care Practitioners Executive Committee.
