In this episode, the team from the Henry Ford Health System to discuss their award winning submission to the ASHP Best Practices. They will share how the addition of a pharmacist to the multidisciplinary interstitial lung disease (ILD) team, along with the robust collaboration with a specialty pharmacy, improved the quality of patient care in ILD and generated significant revenue for their health system.

SPEAKERS

Krishna Thavarajah Krishna Thavarajah, MD, MS (she/her/hers) a Michigan native, joined Henry Ford Hospital in 2009 after completing her Internal Medicine Residency at Boston Medical Center and Pulmonary & Critical Care Fellowship at Northwestern Memorial Hospital. Dr. Thavarajah created and developed the Henry Ford Hospital Interstitial Lung Disease program with the goal of serving as a Detroit-based resource to patients, future pulmonologists and colleagues. The ILD program achieved recognition as a Pulmonary Fibrosis Foundation Care Center in 2018.

Amber Lanae MartirosovAmber Lanae Martirosov, PharmD, MSc, BCPS, BCACP (she/her) is an associate clinical professor of pharmacy practice at Wayne State University Eugene Applebaum College of Pharmacy and Health Sciences. She is also a Clinical Specialist of Ambulatory Care and pulmonary medicine at Henry Ford Hospital. Dr. Martirosov designed and implemented an ambulatory care referral clinic focused on managing high-risk pulmonary patients including those with asthma/COPD, ILD, and PAH. The clinic provides comprehensive therapy management, medication access/ adherence assistance, and patient education. 

Hannah FerrariHannah Marie Ferrari, PharmD, BCACP (she/her/hers) is a clinical assistant professor at Wayne State University Eugene Applebaum College of Pharmacy and Health Sciences. Within this role she serves as an ambulatory care pharmacy specialist at Henry Ford Hospital in the department of cardiology, where she utilizes a collaborative practice agreement to manage cardiovascular diseases, including congestive heart failure and dyslipidemia.

Amanda SoyadAmanda Soyad, PharmD (she/her/hers) is a manager of clinical services at the Henry Ford Health Specialty Pharmacy, Pharmacy Advantage. Dr. Soyad earned her Doctor of Pharmacy degree at Wayne State University in 2016 and has since been dedicated to advancing patient care services to those with chronic and complex conditions. She has a passion teaching her team of pharmacists, technicians, interns and students, and also serves as the residency program director for the Henry Ford Health Community-Based pharmacy residency program.

Vicki BasalygaVicki Basalyga, PharmD, MAS, BCPS, BCPPS is the director of member relations for the Clinical Specialists and Scientists at ASHP. In addition to her director role, Vicki assists in clinical policy development as the secretary for the Council on Therapeutics, serves as a representative for ASHP on numerous national clinical initiatives and works with members to create essential tools and resources for leaders, practitioners, and educators.
